Mission City Community Network (MCCN), a nonprofit 501(c)(3) corporation, has a 30-year history of providing culturally competent and age-appropriate medical, dental, and mental health services to low-income and medically underserved residents of Southern California. MCCN is renowned and respected for delivering comprehensive healthcare that is of the highest quality in a culturally sensitive and linguistically appropriate manner. MCCN provides healthcare services on an open-access basis to all families and individuals seeking health services. Services are provided regardless of race, group affiliation, age, gender, or the patient’s ability to pay.
MCCN operates under a Patient Centered Medical Home (PCMH) model of service delivery. The American Academy of Family Physicians define a PCMH as “an approach to providing comprehensive primary care for children, youth and adults” in “a healthcare setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family.” The model follows a “whole person approach” to healthcare and includes all stages of life, integrated and coordinated care across a complex health and social services system, quality and safety, and enhanced access to care. The patient is empowered with knowledge and resources that will allow them to take an active role in their health and make informed decisions about their well-being. The PCMH model also ensures that all in need of services will be able to access prompt, high quality, affordable, culturally appropriate, and comprehensive healthcare regardless of income level or ability to pay, race or ethnicity, gender, sexual orientation, or any other factor. MCCN recognizes that PCMH practices have been proven to improve patient health outcomes and providers at each MCCN clinic are being trained in PCMH model implementation. MCCN’s Board of Directors has agreed upon this model, as it corresponds with the organization’s mission “to provide, advocate and model quality health care and social services for uninsured and low-income families.” MCCN’s service delivery approach is broad in scope, addressing the medical, oral health, behavioral health, and social complexities encountered by general community residents through a range of primary care services, chronic care/disease management, case management, mental health, and referrals to dental care. The clinics are staffed with family practitioners, internists, psychiatrists, psychologists, mid-level providers, laboratory specialists, patient education specialists, outreach staff, and clinical support staff sufficient to meeting the needs of each service area population. In addition, the Board has adopted the “triple aim,” described by the U.S. Department of Health and Human Services to improve patient care and the patient experience, improve patient health outcomes, and improve cost efficiency. MCCN services are designed to provide better care through continually working to improve the overall quality of care by making healthcare services patient-centered, reliable, accessible, and safe. To improve the health of its patient population, MCCN implements proven practices and interventions that address behavioral, social, and environmental influences in its service areas. Lastly, MCCN strives to contain costs and provide affordable care—including free and low-cost services—to its patients through constant monitoring of financial performance measures.

MCCN participates as a healthcare provider in several programs for low-income individuals and families. Former Medi-CAL recipients and other low-income persons are encouraged to inquire about these special programs.
Mission City Community Network serves the low-income and uninsured populations in the San Fernando Valley, South-bay Region, San Gabriel Valley, and San Bernardino Valley.
