Mission City Community Network (MCCN) is a nonprofit, community-based healthcare organization dedicated to delivering high-quality, compassionate, and culturally sensitive health services to underserved populations across Los Angeles County and beyond.
Founded in 2000, MCCN has grown from a small community clinic into a trusted network of Federally Qualified Health Centers (FQHCs). We proudly serve thousands of individuals and families each year—regardless of their income, immigration status, or ability to pay. MCCN receives HHS funding and holds Federal Public Health Service (PHS) deemed status for certain health related claims, including medical malpractice coverage for the organization and its covered individuals.
Our approach to care is holistic, patient-centered, and deeply rooted in community trust. We offer a comprehensive range of services including primary care, mental health, dental care, chronic disease management, preventive screenings, and support services, all tailored to meet the diverse needs of the communities we serve.

Mission City Community Network (MCCN) is a nonprofit 501(c)(3) corporation with over 30 years of experience in delivering compassionate, high-quality healthcare to the uninsured, underinsured, and medically underserved residents of Southern California. Since its inception, MCCN has earned a reputation of providing culturally competent, age-appropriate medical, dental, and mental health services in a linguistically inclusive and respectful manner.
At the heart of MCCN’s service philosophy is a commitment to equitable access. Healthcare services are available to all individuals and families regardless of income, race, gender, age, ability to pay, or background through an open-access model that ensures dignity, safety, and respect for every patient.
MCCN operates under the Patient-Centered Medical Home (PCMH) model of care. Defined by the American Academy of Family Physicians the PCMH is a holistic approach to healthcare that fosters strong partnerships between patients and their care teams. It ensures comprehensive, coordinated, and continuous care that empowers individuals with the tools and support needed to actively manage their health and well-being. Through this model, MCCN addresses a wide range of needs, including medical, oral, and behavioral health, chronic disease management; mental health counseling; and dental care. MCCN’s clinics are staffed by dedicated family practitioners, internists, psychiatrists, specialists, and outreach teams working collaboratively to provide integrated, high-quality care tailored to the unique needs of each community.
In alignment with the “Triple Aim” framework developed by the U.S. Department of Health and Human Services, MCCN is committed to:

The organization continuously refines its practices to improve health outcomes, adopt evidence-based interventions, and address the behavioral, social, and environmental determinants of health.
