It is committed to empowering patients We provide materials and support on chronic disease care, preventive screenings, immunizations, and personalized health topics delivered by bilingual, culturally competent staff.

Our onsite health education room hosts regular workshops an individual counseling session focused on topics such as diabetes prevention, heart health, weight management, and balanced nutrition. These trainings are led by bi-lingual educators and designed to support patients in making informed choices for their well-being.

At Mission City Community Network, we believe cost should never be a barrier to receiving quality healthcare. That’s why we are dedicated to helping individuals and families understand and access the insurance coverage that works best for them. Our knowledgeable team walks patients through the entire process — from learning about available plans to enrolling in coverage and making updates when needed.

We assist with Medi-Cal, Covered California, Medicare, and other private health insurance plans, ensuring you know your options and can choose what fits your needs. If you already have insurance but need to make changes, such as selecting or updating your Primary Care Provider (PCP), we can help guide you step by step, so your care is seamless and uninterrupted.

For those who are uninsured or underinsured, we offer a Sliding Fee Scale based on household size and income. This program is designed to make healthcare affordable, so everyone in our community, regardless of financial status can receive the care they deserve.
