Mission City Community Network’s Online Patient Portal gives you convenient, secure access to your personal health information, anytime, anywhere. Whether you are checking lab results, or, messaging your healthcare provider, it’s all just a few clicks away.
What You Can Do on the Patient Portal
• View lab results and medical history.
• Schedule or request appointments.
• Request prescription refills.
• Send and receive secure messages from your care team.
• Update personal and insurance information.
• Access visit summaries and educational materials.

Your privacy is our priority. The portal is protected with advanced security protocols, keeping your medical information confidential and accessible only to you, or those you choose to share it with.
Can Other View My Info?
By default, only you can view your medical information through the patient portal. However, you may choose to grant access to a trusted family member or caregiver.
To share access:
• Log into the Patient Portal
• Go to “Portal Access”.
• Click “Permit Access” and enter the person’s information.
You can revoke this access anytime. Once authorized, the individual will be able to view your records and upcoming visit summaries unless you remove their access.
Sharing & Downloading Records:
You may also:
• Download your medical documents.
• Share your care summary, test results, or medication list with your physician or caregiver.
• Use certified direct messaging (when available) to securely email records to your provider.
You are in control of your health information – safe, private, and shareable only with your permission.
How to Sign Up:
It’s quick and easy to get started.
Option 1: During Your Visit
Ask our front desk staff to enroll you during your appointment. You will receive a secure link to create your login.
Option 2: Online Enrollment
Visit mccn.org/patient-portal and click “Sign Up Now”. Follow the prompts to verify your identity and set up your account.
